ตรวจหาเชื้อ H.Pylori (ด้วยวิธีเป่าลมหายใจ Urea Breath Test)

Detecting Helicobacter pylori Infection Without Endoscopy

Helicobacter pylori (H. pylori) is a type of bacteria that can live in the stomach. H. pylori infection is associated with gastritis, gastric and duodenal ulcers, and an increased risk of stomach cancer.

Testing for H. pylori may be recommended for individuals with symptoms suggestive of stomach or upper gastrointestinal problems, as well as those who require follow-up testing after H. pylori treatment.

One of the commonly used methods for detecting H. pylori is the Urea Breath Test (UBT), a non-invasive test that does not require an endoscopy.

What Is a Urea Breath Test?

The Urea Breath Test (UBT) is a non-invasive test used to detect an active H. pylori infection.

H. pylori produces an enzyme called urease, which breaks down urea and produces carbon dioxide. During the test, the patient takes a capsule containing urea. If H. pylori is present in the stomach, the urease enzyme produced by the bacteria breaks down the urea, resulting in an increase in detectable carbon dioxide in the patient’s breath.

Breath samples are collected before and after taking the urea capsule. The samples are then analyzed and compared to determine whether H. pylori is present.

Because the test uses a breath sample, no endoscopy or stomach tissue biopsy is required.

How Is the Urea Breath Test Performed?

The Urea Breath Test is a simple, non-invasive procedure that typically takes approximately 20 minutes.

1. Initial Breath Sample

The patient breathes into a collection bag to provide an initial breath sample before taking the test substance.

2. Take the Urea Capsule

The patient takes a capsule containing urea and drinks one glass of water.

3. Second Breath Sample

After approximately 20 minutes, the patient provides a second breath sample. The two samples are then analyzed and compared to determine whether H. pylori infection is present.

Benefits of the Urea Breath Test

The Urea Breath Test offers several benefits:

  • Non-invasive – no endoscopy is required
  • No stomach tissue biopsy is needed
  • Simple and convenient procedure
  • Can be used to detect active H. pylori infection
  • Can be used as a test of eradication after H. pylori treatment to determine whether the infection has been successfully eliminated

How Should You Prepare for the Test?

Proper preparation is important because certain medications may affect the accuracy of the Urea Breath Test.

Medications that may interfere with the test include:

  • Antibiotics
  • Proton Pump Inhibitors (PPIs)
  • Potassium-Competitive Acid Blockers (PCABs)

Depending on the medication, you may need to stop taking certain medicines for a specific period before the test.

Do not stop taking any medication on your own. Please inform your doctor or healthcare staff about all medications you are currently taking. They will advise you on the appropriate preparation and whether any medications should be temporarily discontinued before testing.

Who Should Consider H. pylori Testing?

Your doctor may recommend H. pylori testing if you:

  • Experience upper abdominal or epigastric pain or burning
  • Frequently have indigestion, bloating, or abdominal discomfort
  • Have a history of gastric or duodenal ulcers
  • Have previously been diagnosed with H. pylori infection and need follow-up testing after treatment

The need for testing will depend on your symptoms, medical history, and your doctor’s assessment.

Urea Breath Test vs. Upper Gastrointestinal Endoscopy

The Urea Breath Test and upper gastrointestinal endoscopy (EGD) serve different purposes.

The Urea Breath Test detects active H. pylori infection through a breath sample without inserting an endoscope into the digestive tract. However, it cannot directly visualize the stomach or identify structural abnormalities or lesions.

An upper gastrointestinal endoscopy allows a doctor to directly examine the esophagus, stomach, and duodenum. During the procedure, tissue samples can also be collected for further examination when necessary.

Therefore, the most appropriate test depends on your symptoms, medical history, and individual risk factors. Your doctor will determine which examination is most suitable for you.

Why Choose a Urea Breath Test?

The Urea Breath Test is a convenient and non-invasive option for detecting active H. pylori infection without the need for endoscopy.

It may also be used to confirm whether H. pylori has been successfully eradicated following treatment, as recommended by a physician.

If you experience recurring stomach discomfort, indigestion, or upper abdominal pain, consult a doctor to determine whether H. pylori testing is appropriate for you.
